If you think you qualify, download and submit a Self Certification Mailer (see below), available in PDF format. The mailer will print out according to postal regulations if printed double-sided on Legal-size paper.
In addition to the self certification mailer, IID requires REAP participants to fill out and complete IRS Form 4506-T (Request for Transcript of Tax Return).
