MEDICAL

IID provides a self-funded Group Medical Benefits Plan for active, full-time, regular employees and  pays a large part of the costs for dependents. 

Our Claims Administrator is, Principal Life Insurance Company.  IID contracts with Principal to review, process, and pay medical claims on our behalf.  All claims dollars spent come directly from IID funds.

In addition to the Medical Benefits Plan, a prescription card plan, mail-order prescription plan, employee assistance plan, and Mexico PPO plan are provided.  

IID has several Meet and Confer units and if the benefit level is different for any of those units, the difference will be identified specifically for each unit.

Deductibles:

Individual Stop Loss:                                                                
Plan will pay 100% of usual and customary covered medical expenses over $10,000, excluding mental health and prescription expenses. 

Maximum:                                                                                 
Per individual, per lifetime...................$2,000,000            
Acupuncture, per calendar year...........$2,500     
Chiropractic, per calendar year............$2,500 


Routine physical exams:      

  •  PSA (Professional Salaried Association) covered up to $2,000 per employee and $1,000 per dependent. 

All other employee units covered up to $1,000 per employee and $1,000 per dependent.  

Preferred Provider Organization (PPO)  
IID offers a PPO option.  If a PPO provider is used your covered medical expenses will be reimbursed at 90%.  If a non-PPO provider is used your covered medical expenses will be reimbursed at 70%.  If you move out of the area and there is no PPO provider readily available, you will be reimbursed at the non-PPO provider 70% rate. 

We no longer distribute PPO provider directories.  It is difficult for Principal and MultiPlan to keep the information up-to-date.  We encourage you to periodically contact Principal to verify your doctor’s PPO status.
