Your Responsibility:

  1.  Keep in touch with your immediate supervisor.  Let them know how you are doing on a weekly basis.
  2.  Turn all of your treating physicians’ notes in to the Employee Benefits Services Section.  Our disability management professionals will provide your supervisor with the information they need to know. Do not give your supervisor your treating physician’s notes or copies of your appointment cards.
  3.  It is extremely important that you follow all the instructions provided by your treating physician and keep all of your scheduled appointments so that you can make a full recovery. 
  4. If your doctor informs you that you may return to work without limitations or restrictions, provide the doctor’s note to disability management in the Employee Benefits Services office, immediately.  To expedite this process, the doctor’s office may fax a copy of the release information directly to our office: (760) 339-9771. 

    We will not accept faxed doctor’s notes from you.
  5.  If your treating doctor informs you that you may return to work with temporary limitations or restrictions, make sure they identify what you can do as well as what you cannot do and the specific period of time the limitations or restrictions may apply.  Again, the doctor’s office may fax a copy of this information directly to our office: (760) 339-9771.
  6. Your participation in our interactive return-to-work meetings is vital.  You are an important part of the team.  The insight you can provide regarding your job duties assists in the team understanding how to temporarily modify your regular job or what type of temporary assignment in another work area may be appropriate for you.
  7.  If you return to work with limitations or restrictions and experience discomfort, it is your responsibility to inform your supervisor and schedule an appointment with your treating doctor and to address your medical issues. *Keep in mind, that it is YOUR responsibility to provide IID with sufficient medical information—it is not the responsibility of the treating physician, the insurance carrier or the insurance administrator. Also, doctor’s notes are to be provided after each visit to your treating physician or not longer than every thirty (30) days of absence.
